H3X Inc ("H3X," "we," "us") provides a healthcare management platform for clinics and their patients. This Privacy Policy explains what information we collect, how we use it, and the choices you have. We do not sell your personal information.
1. Information we collect
We collect information you or your clinic provide (such as name, contact details, mobile phone number, and appointment information) and information generated when you use the platform. Health information handled on behalf of a clinic is processed under our Business Associate Agreement and applicable law, including HIPAA.
2. How we use information
We use information to operate the platform, coordinate care, send appointment reminders and messages you have requested, provide support, and maintain security.
3. SMS / Text messaging
We collect mobile phone numbers and SMS consent solely to communicate with patients about their care, such as appointment reminders, scheduling updates, and messages from the care team.
Text messaging is optional. Consenting to receive text messages is not required to receive services, create an account, book an appointment, or complete any transaction. Opt-in is a separate, voluntary action.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text-messaging opt-in data and consent are not shared with any third parties. We only share the minimum information necessary with our messaging provider to deliver the messages you have requested.
Message frequency varies. Message and data rates may apply. Reply STOP at any time to opt out, or HELP for help. Opting out of text messages does not affect your care or your ability to use H3X.
4. How we share information
We share information with service providers who help us operate the platform (such as hosting and messaging providers) under contractual confidentiality obligations, with your clinic, and where required by law. We do not sell your data to any third party.
5. Data security
We use administrative, technical, and physical safeguards designed to protect your information. No method of transmission or storage is completely secure, but we work to protect your information and continuously improve our safeguards.
6. Your choices
You may opt out of text messages at any time by replying STOP. You may request access to or correction of your information by contacting us. Patients should also contact their clinic regarding their health records.
